Office Policies

Thank you for scheduling your appointment with us. We strive for transparency, so we ask that you please take a moment to review our office policies below before completing your paperwork.

REGISTRATION POLICY

All patients must complete their Patient Registration via the link at the bottom of this email based on the following timeline:

  • Same-day appointments: Within 2 hours of booking.
  • Appointments less than 3 days away: Within 8 hours of booking.
  • Appointments 3 or more days away: At least 3 days before your appointment.

*If your forms are not submitted within these windows, we will unfortunately be unable to hold your appointment, and it may be canceled. 

INSURANCE INFORMATION

Occasionally, the insurance information provided may be incorrect, or we may need additional details to verify your benefits. If we reach out regarding your insurance, please respond as soon as possible.

*We reserve the right to cancel any new patient appointment if proper coverage information is not provided.

APPOINTMENT CONFIRMATION POLICY

All appointments have to be confirmed at least 72 hours prior to the scheduled time.

*We reserve the right to cancel any appointment that is not confirmed. 

GRACE PERIOD (10 Minutes)

We understand that unforeseen circumstances can delay your arrival. However, to ensure all patients are seen on schedule, we strictly adhere to a 10-minute grace period and cannot extend this courtesy further.

X-RAY POLICY

Dental X-rays are necessary for all exams, consultations, and dental treatments. If your insurance limits the frequency of X-rays, you may be asked to provide recent X-rays from another office or pay for new ones out of pocket. We cannot proceed with your visit without X-rays on file.

Furthermore, because our hygienists operate under the supervision of our dentists, we require all patients to undergo a dental exam and X-rays once a year.

CANCELLATION POLICY

Your appointment time is reserved exclusively for you. Therefore, we require at least 24 hours’ notice if you need to cancel.

Cancellations made with less than 24 hours’ notice, as well as no-shows (missing an appointment without calling), are subject to a Broken Appointment Fee:

  • 1st Missed Appointment: Fee waived as a courtesy.
  • 2nd Missed Appointment: $75 fee.
  • 3rd Consecutive Missed Appointment: $150 fee.

Please be advised that we cannot reschedule your appointment until any outstanding cancellation fees are paid. We reserve the right to dismiss patients from the practice if three consecutive appointments are missed.
